Lactation Complications: Understanding, Overcoming, and Getting Support
For many new parents, breastfeeding is imagined as a serene, natural experience—baby nestled in your arms, feeding with ease. While this can be true for some, the reality is that many parents face challenges along the way. Lactation complications are common, and they don’t mean you’re doing anything wrong.
As a doula, my role is to remind you that you are not alone, your struggles are valid, and there is help available.
Common Lactation Complications
Understanding the most frequent breastfeeding challenges can help you spot them early and seek support quickly.
1. Latching Difficulties
A shallow or improper latch can cause pain, poor milk transfer, and frustration for both you and your baby. This can be due to positioning, anatomy differences (like tongue tie or lip tie), or simply the learning curve of feeding.
2. Low Milk Supply
Some parents worry their milk isn’t enough. True low supply can happen, but sometimes it’s a perception issue—baby may be getting more than you realize. Causes can include infrequent feedings, certain medications, or hormonal conditions.
3. Oversupply and Fast Letdown
Too much milk or a strong letdown reflex can cause choking, coughing, or gassiness in babies. It can also lead to engorgement and discomfort for the parent.
4. Engorgement
When breasts become overly full, they may feel hard, painful, and difficult for the baby to latch onto. Engorgement can happen in the early days when milk first comes in or after skipped feedings.
5. Mastitis
A painful breast infection that often comes with swelling, redness, fever, and flu-like symptoms. It can develop from blocked milk ducts or unresolved engorgement.
6. Nipple Pain or Cracking
While some tenderness is normal at first, ongoing pain or damage may signal an issue with latch, positioning, or infection such as thrush.
The Emotional Impact of Lactation Challenges
Beyond the physical discomfort, lactation complications can bring feelings of guilt, frustration, and even grief when breastfeeding doesn’t go as planned. It’s important to remember: feeding your baby is not a measure of your worth as a parent.
Support, understanding, and timely guidance can make a world of difference in how you navigate these challenges.
How a Doula Can Support You Through Lactation Complications
While a lactation consultant (IBCLC) is the clinical expert in diagnosing and treating breastfeeding problems, a doula can play a vital role in your support team:
Observation & Encouragement: Helping you recognize feeding cues and guiding you through different holds and positions.
Comfort Strategies: Suggesting tools like warm compresses, breast massage, or pumping plans to relieve discomfort.
Referrals: Connecting you with trusted lactation consultants, pediatricians, or support groups when needed.
Emotional Support: Being a calm, reassuring presence when challenges feel overwhelming.
Education: Offering evidence-based resources so you can make informed decisions about your feeding journey.
Tips for Navigating Lactation Complications
Act Early. Don’t wait for pain or problems to worsen—seek help as soon as you notice an issue.
Feed Frequently. Regular feeding or pumping helps maintain milk flow and reduce engorgement.
Stay Hydrated & Nourished. Your body needs fuel to produce milk and recover from birth.
Use Proper Positioning. Experiment with holds like the football or side-lying position to find comfort.
Give Yourself Grace. This is a learning process for both you and your baby.
You Don’t Have to Do This Alone
Lactation complications can feel discouraging, but with the right support, most challenges can be overcome—or worked around in a way that keeps you and your baby happy and healthy.
